Please note that all HMO plans require a pre-authorization referral from your Primary Care Physician (PCP) to be received at our office prior to your appointment. HMO patients will not be seen without a referral. It is the responsibility of the patient to verify whether their plan requires a pre-authorization referral prior to seeing a specialis.
